At the time of your initial visit, your treatment plan will be established and an estimate of fees will be provided to you. Payment is due at the time of service. Patients Using Insurance:
If you plan to use your insurance for part of your payment, we will file your claims for you as a courtesy. In the event an insurance carrier has not paid within 90 days of billing, any unpaid professional fees are due and payable in full from you.
Returned Check Fee:
A fee of $20.00 will be charged for any check returned for non-sufficient funds. In the event that a check is returned for non-sufficient funds, all future payments will be required via cash or credit card at the time of service.
